Dental Insurance
  • Together with Administaff, o3 has dental benefits covering a wide range of dental procedures, including preventive and diagnostic service, basic (restorative) services, major (prosthodontic) services and orthodontia services.
Vision
  • Vision benefits cover eye examinations, prescription lenses, eyeglass frames and contact lenses. You can use any optometrist or a provider who participates in the VSP provider network for greater benefits and less out of pocket.
Life & Personal Accident (AD&D) Insurance (CIGNA)
  • Employee only; premiums paid by Administaff; available to full-time active employees only if medical coverage is elected.
  • Basic Term Life Insurance 
    Amount equal to 1x employee’s base annual earnings* $15,000 minimum $50,000 maximum.
  • Basic Personal Accident Insurance 
    Amount equal to 1x employee’s base annual earnings* $15,000 minimum $50,000 maximum.
    * Base annual earnings include your base annual earnings paid by Administaff,  
    excluding bonuses and overtime, but including any commissions. Coverage  
    amounts based on your base annual earnings are rounded to the next higher  
    $1,000. 
  • Voluntary Group Universal Life Insurance (CIGNA)*
    Optional coverage for full-time active employees and their families; premiums paid by
